CHMS Boys Basketball Summer Tryouts

CHMS BOYS BASKETBALL WILL BE HAVING SUMMER TRYOUTS FOR RISING 6TH, 7TH AND 8TH GRADE BOYS ON TUE MAY 31ST AND WED JUNE 1ST FROM 315-500 @ CHMS. 

ALL PLAYERS MUST HAVE PHYSICALS, CARDIAC ARREST, CONCUSSION AND CONSENT TO TREAT FORMS FILLED OUT AND ON FILE IN THE CHURCH HILL MIDDLE SCHOOL OFFICE, ON MAY 27TH, BEFORE YOU TRY OUT. IF YOU ARE A 6TH GRADER, YOU MUST TURN YOURS IN TO THE CHURCH HILL MIDDLE SCHOOL OFFICE, NOT THE INTERMEDIATE SCHOOL. THERE ARE LINKS ON THE SCHOOL WEBSITE TO ACCESS THESE FORMS OR YOU CAN GET THEM AT THE SCHOOL. 

MAKING THIS TEAM DOES NOT GUARANTEE THEM A SPOT ON THE ACTUAL SCHOOL TEAM IN THE FALL. PLAYERS NEED TO BRING THEIR OWN WATER TO TRYOUTS AS WELL. 

DON’T FORGET ALL PAPERWORK MUST BE ON FILE IN THE CHURCH HILL MIDDLE SCHOOL OFFICE BY MAY 27TH OR YOU CANNOT TRYOUT. 

CHMS Sports Physicals Information Summer is just around the corner. The 2022/23 school year will be here before we know it. If you have a student in your household who will be participating in a sport at CHMS (or VHS) next year, there are some important things you will need to know.

  • Each athlete MUST have a completed physical on file in the CHMS office BEFORE they are allowed to try out for a sport.

  • The physical MUST be dated AFTER April 15, 2022.

  • Students are not allowed to participate in any way until we have the physical on file.

  • Physical forms can be found in the offices at CHMS and CHIS and may also be downloaded from the CHMS website. (Scroll to the bottom of the home page and download/print the PDF.)

  • For players participating in sports this summer, physicals must be on file NO LATER THAN May 27, 2022 . There are three ways to obtain a sports physical-

  • See your primary care physician. Make sure this is AFTER April 15.

  • Take a full copy of the physical form to Fast Pace Health in Church Hill (Food Lion parking lot) and get a physical done there. The cost will be $25, no appointment needed. If you wear contacts/glasses, make sure to bring those for the vision test.

  • Rural Health will be at CHMS on May 5th and CHIS on May 10th to give physicals. The cost will be $10. Please make plans accordingly, as these dates are approaching fast. We look forward to having your child play sports at CHMS in 2022/23
